Carlebach and revoking smicha

Question

There has been some recent discussion on Carlebach, so I thought I would ask some related questions: Can the Smicha given today be revoked by the person who gives it? I've heard that Rav Aaron Kotler gave smicha to Shlomo Carlebach before he left to Lubavitch and that Rav Kotler later revoked Carlebach's smicha publicly because he had gone off the derech. I've only heard of 2 other cases where this happened. One in which the Satmar Rebbe revoked a students' Smicha and a more recent case in Israel that is not public. Can you confirm the Carlebach story and that this is possible? I have some additional questions on this: 1) Is it appropriate for people to continue referring to Carlebach as a Rabbi or allow people to do so (or did he have another smicha? I've never heard of another smicha.). 2) If a person's smicha is revoked, does that mean that any smicha he has given loses it's legitamacy?
